Dementia support
ARE YOU WORRIED ABOUT YOUR MEMORY?
Many people live with the anxiety that something is wrong with their memory, their thinking or judgement. If this is you, or someone you know, please contact your GP practice.
They will complete some preliminary investigations to determine whether there may be underlying physical health reasons for your memory problems. If appropriate, the GP will refer you to a local memory service.
Once they have made the referral you will be invited to a memory clinic, or an appointment can be arranged in your own home if it is difficult for you to attend a clinic.
Following the appointment, you will be put in touch with a support worker who will meet you and they will discuss any practical concerns you may have.
The support following a diagnosis of dementia includes:
- Information regarding the type of dementia you may have
- Links and contacts to local organisations who will offer you practical support and the opportunity to meet other people who may have received a diagnosis of dementia
- Benefit advice
- Information about the lasting power of attorney
- Helpful suggestions for planning for the future
- Adaptations to your home you may require
If you receive a diagnosis of dementia, it is important to plan for the future. People who have been in this position often say how they have felt empowered when offered the opportunity to plan for their future, and what a positive difference it has made to their day to day living.
